Immunizations

What are the Immunization Requirements?

All students are required to provide proof of: 

  • Three-dose series of DTaP, or TD, with one dose being Tdap and one dose having been administered within the past 10 years. Given in 1-6 months intervals.
  • Two-dose series of MMR (measles-mumps-rubella combined) given in 1- month intervals. Vaccine given prior to turning the age of one will not be accepted.
  • One meningococcal vaccine (if you are between the ages of 16 and 22)

International students, in addition, must have:

  • One QuantiFERON or T-SPOT鈥(亚色影库 requirement)鈥攖est should be taken after July 21, test prior to this date will not be acceptable.

Where Can I Go to Get Any Missing Vaccines?

Vaccines are given at most walk-in, take-care clinics. Your student health insurance will cover the full cost.

Submitting My Immunization Records

The Student Health and Wellness Center utilizes Med + Proctor, an electronic vaccination documentation collection platform, as our innovative strategy to monitor and collect your immunization information. As you know, Illinois state law requires documentation of vaccinations when entering a secondary institution. Med + Proctor is HIPAA- and FERPA-compliant.

This system for monitoring immunization compliance will streamline your immunization compliance process and help you avoid registration holds, so you can focus on other important matters as you prepare for a successful academic journey at 亚色影库.

For your convenience and to ensure you have complete control of your medical records, there is a one-time $10.00 fee to access Med + Proctor鈥檚 services, which includes:

  • 48-Hour Turnaround Time on document review
  • Epic and State Immunization Registry Search for faster verification
  • Lifetime Access to Your Immunization Records, even after graduation

Through this process your documentation is handled efficiently and stays accessible whenever you need it鈥攚hether for study abroad, employment, or other health records needs.

How to Use Med + Proctor

  1. First, set up your hawk.ILTech.edu email account.
  2. Go to the 亚色影库 Portal and log into your account.
  3. Search for 鈥淢ed + Proctor鈥 on the 亚色影库 Portal to register and start a new account.
  4. Follow the on-screen instructions.  

Upload your completed immunization forms, which must be stamped and signed by your medical provider.

To ensure efficient processing, please do not bring or email your forms to the Student Health and Wellness Center. All forms must be uploaded online to Med + Proctor.

Once submitted, your documents will be reviewed for compliance. You will receive updates and any action items via your 亚色影库 Portal email. It鈥檚 important to regularly check your email to ensure you stay in compliance and avoid holds.

You can also monitor your account by clicking on the STATUS or MESSAGE tab at the top of your Med + Proctor page.

We encourage you to submit your information as soon as possible. Verification of documentation typically takes 48 hours. However, holds may take up to 3-5 business days to be removed after submission.

Prior to Coming to the SHWC for Your Immunizations

If your insurance card is not available, you will need to wait until it is available to come in for any vaccines.

Immunization Policy

  1. Students are expected to update their immunization records in no later than the first week of classes.
  2. A registration hold will be placed on your account if there is not a valid or updated Immunization Verification Form on file for you and/or if you have been deemed "non-compliant." The hold will be active on the last date of the add/drop period.
  3. Registration holds will be lifted once you are determined to be in compliance with  and the Illinois Tech immunization requirements.
